Below is the AVP’s On-Line Incident Report Form. If you have experienced or witnessed any homophobic abuse or violence you can report it to the AVP using this on-line form. Do not fill in this form if you have previously reported the incident to the AVP.Be as accurate and as detailed as possible but if you don’t know an answer leave it blank.
